Enrollment Registration Form 2008-09

Note: Filling out this form is for Enrolling your student in our program.  Be prepared to provide payment for your enrolling student either here online or by contacting HPS Prep School at 1-360-695-3343. 

Process or Steps of Registration

  1. HPS Prep School receives an application from the web page, or a request for more information (usually from a phone call).

  2. A Parental Questionnaire and student questionnaire/s according to the grade of the student/s along with a current bulletin is mailed to the family.

  3. When the questionnaires along with the registration fees (by check or bank card) are returned to HPS, a teacher will review the information and make recommendations according to the student's individual needs.  At this time an individualized curriculum plan will also be sent to the parents along with guidelines for getting started and reporting.

  4. The parent then orders the needed books.  This can be done through the mail, or by a bank card through PayPal.  Most orders are sent within a week.  It usually takes three to five working days, depending upon the location, for the books to reach the student.

 *  To shorten the time from application to getting started, you may use a Credit Card through PayPal and e-mail the information needed.  A call to 1-360-695-3343 (voice mail) giving your name, address, phone, and student grade/s will also help.

$279 Grades 7-8 Written Individual Evaluation and Curriculum Preparation, Permanent Record Keeping, Telephone Counseling and Support, One Progress Evaluation, One PASS test, Certificate of Completion
$399 Grades 7-8 Full Program With Quarterly Evaluations Includes Curriculum Recommendations, One Escrow Credit (if qualified) with four written Evaluations, Permanent Record Keeping, Telephone Counseling and Support, One PASS test, and Certificate of Completion.
$495 High School Program for Grades 9-12 includes Individualized Curriculum Recommendations, Permanent Record Keeping, Four Written Progress Evaluations, Diploma (for enrolled seniors meeting all requirements), Transcript--One free--others are $5.  Single Courses also available.
$59 Video conferencing-- one time family fee.  Must have high-speed internet and Digital Video Phone (Must call for current pricing).  This  phone can also serve as your regular phone and has unlimited long distance in the United States, Canada, and Puerto Rico ($28.95).  Call for more information.
$49 Counseling $49. per hour for un-enrolled students desiring guidance.
$140 High School Program (quarterly payment plan) First quarter's payment due at time of registration. Payments due quarterly. (does not include books)
$129 Foreign Students -- fee for additional postage. Fee needed for students not having a stateside address. Students may opt to have books and evaluation mailed to a stateside address and have a contact person be responsible for mailing it to them.
